Client Application
If you or a family member or client/patient would like to become a CHAPS client, please download, print, and fill in the CHAPS Client Application.
We request a physician's release to prove that the prospective client is medically able to ride or participate in equine assisted activities, which is a separate form available at CHAPS or by calling the Director at 307-673-6161.
CHAPS Clients have a common denominator - a special need or disability diagnosed by a mental or physical health care professional, agency, or educator. We encourage each client's support team-family, health care providers, educators, and case workers - to get involved with each client's personal plan. Our first step is filling out the application and returning it to CHAPS at the address provided on the application. After that, if financial aid is not requested, an intake session is scheduled to discover what the client's needs and goals are, and what they hope to accomplish. From there, they are assigned to a Therapy Instructor, Therapy Horse, and appropriate volunteers if needed, and a regular day and time is scheduled for sessions.
